In the Event of Loss, Theft, or Fraud
If you notice a transaction that you do not recognize, please refer to the following steps:
Call US Bank
Immediately call US Bank at 800-344-5696, and report the loss, theft, or fraudulent transaction.
Their agent will close your card and issue a new one to the address you have on file. It can be helpful to have a list of recent transactions ready, so that you can verify which ones are legitimate.
File a Police Report
If the card was stolen or used fraudulently, you must file a Police Report and obtain a report case number. Please refer to the following contact information:

Complete a Loss, Theft, or Fraud Form
Complete a Loss, Theft, or Fraud Form and submit it to Berkeley Card Program at creditcard@berkeley.edu. The Police Report Case Number must be included on this form. Your new card cannot be activated until this form has been submitted.

Disputes
Contact the supplier and attempt to resolve the dispute.
If you are unable to reach a resolution with the supplier, call US Bank at 800-344-5696 no later than 60 days after the transaction date to open a dispute case.
Often at the beginning of the process, US Bank will issue a credit for the amount you are disputing, you will still need to complete their full dispute process.
US Bank will notify you in writing of the results of its investigation.
If the merchant proves it was a legitimate business transaction, US Bank will charge the amount back to your card for the credit that they had issued previously, otherwise the credit will remain to cancel out the disputed transaction.
